This form is for organizers of special events and markets. If you are a food vendor for a special event or market, please fill out the “Food Vendor Application Form”. Please note, this application must be submitted 30 days prior to the event.

(Each booth is required to have a handwashing station:  One (1) handwashing station may be shared between two (2) food booths providing that they are located directly next to another)

Note:  Special Events/Markets that are not exempt from the Food Premises Regulation will NOT be permitted to sell food prepared at an uninspected premises.

Please provide a diagram that indicates the location of the food booths, handwashing stations, garbage receptacles, dishwashing area, waste water disposal, washrooms and fencing.
