Your Child’s Immunization Record

Families should keep track of their child’s immunizations. In Ontario, it is the parent/guardian’s job to keep up-to-date records and to report their child’s immunizations to their local Public Health Unit.
As a parent/guardian, you can keep your child(ren) healthy with routine vaccinations. Health care providers, including doctors, are not required to report immunizations to the local Public Health Unit.
Any time your child receives an immunization, please update your child’s vaccine record online at immune.wechu.org. This website, also known as the Immunization Connect Ontario (ICON) portal, is a provincial database for immunizations. Once you have submitted your child’s immunization record through ICON, it will be processed by WECHU staff within several business days. Log back in to the portal and check to ensure your child’s record has been updated. Continue to use the ICON portal regularly to access your record when needed, report immunizations, and check if any immunizations are due.
How do I report vaccinations to the WECHU and/or access my child’s immunization record?
- To view your child’s immunization record online, visit immune.wechu.org. This will only show you which vaccines have been reported to the WECHU. You may need to update your child’s immunization record, or request for your health care provider to send records to the WECHU, if your child received vaccines that are not listed on their online immunization record.
- To report your child’s vaccinations to the Health Unit:
- Visit immune.wechu.org
- Fax 519-258-7288
- Call 519-258-2146, ext. 1222
- Bring your child’s records to our Windsor or Leamington offices.
How do you request a paper copy of your own or your child’s immunization record from the Health Unit?
You may choose to obtain a paper copy of your or your child’s record through one of the following options:
- Visit immune.wechu.org to see immunization record on file. You can download a copy and print it whenever you need it. This is the fastest and easiest way to access your record.
- Visit the WECHU’s Windsor or Leamington office. Be sure to bring proof of identity.
- Fill out the online Freedom of Information Request form. You will be required to provide identification and proof of authorization. A secure digital file will be shared that you may print.
What if I choose not to immunize my child?
Under the Immunization of School Pupils Act R.R.O. 1990, Reg. 645, students must be immunized against meningococcal, measles, mumps, rubella, diphtheria, tetanus, pertussis, polio and varicella (for those born on or after January 1, 2010) or have a valid Medical, or Conscience or Religious Belief exemption on file at the Health Unit.
